Low-dose anticoagulation for peripheral deep vein thrombosis of the lower extremities in cancer patients

Abstract Background Venous thromboembolism (VTE) is a frequent complication in cancer patients and often detected as asymptomatic peripheral deep vein thrombosis (DVT). However, guidelines for DVT have not been established either general cardiology or onco-cardiology. Furthermore, patients, it generally considered difficult to administer standard doses of anticoagulants due the high risk bleeding caused by thrombocytopenic tendency during chemotherapy, well lesions such gastrointestinal gynecological cancers. Treatment with direct oral (DOACs) may minimize worsening prevent transition proximal DVT. Objective The purpose this study was investigate optimal treatment patients. Methods 242 consecutive lower extremities (excluding complications pulmonary DVT) were examined venous ultrasound from April 2019 March 2020. 190 could be followed up examination, 2 years. course retrospectively examined. In cases anticoagulant therapy, low dosage DOACs evaluated. Results 142 treated (AC) therapy (group AC(+)), 48 without AC AC(−)). (4 warfarin rest DOACs). worsened 8 (5.6%) AC(+) group 23 (47.9%) AC(−) group. significantly less (p<0.0001). There no type 4 (17.4%) Of 138 who used 62 half than dose maintenance (low-dose DOAC group), 76 (standard-dose group). low-dose group, 5 cases, remained unchanged 7 improved 50 cases. standard-dose there DVT, 3 change, 69 improvement. significant difference effect between two groups. thrombus-related deaths. Conclusion Peripheral worsens about anticoagulation, 17% these worsen VTE. addition, are found effective treating can reduce mortality should one supportive treatments completion management.
